Lipedema is a chronic condition in which fatty tissue accumulates unevenly and symmetrically in the legs, arms or other areas of the body. This condition occurs primarily in women and can be genetically determined.

How is lipedema diagnosed?

The diagnosis of lipedema is based on the overall picture of current symptoms, the previous course and physical examination. It is purely clinical – meaning it depends on what we can see, feel and rule out.

What does lipedema look like?

You can often see the typical disproportion – the emphasis of excess volume on the legs and arms, while the torso is often less affected. It appears symmetrical and begins in the legs, usually already above the ankles. On the arms it often begins from the wrists with visible demarcation lines.

What do we feel during examination?

We feel the swollen fatty tissue and its pressure sensitivity. This arises from the combination of pressure and inflammatory processes. As too many fat cells – which can also each become maximally large – fill the typical anatomical spaces such as the inner knees, outer thighs, or triceps area on the back of the upper arm.

What do we rule out?

We must rule out other possible causes, such as varicose veins, which can also promote painful swelling – using ultrasound and Doppler sonography. It is a misconception that lipedema fat cells can be made visible by sonography.

Smooth skin surface, increased fatty tissue palpable, pressure sensitivity. Initial pain possible.

Wavy-nodular skin surface (mattress phenomenon), significant discomfort, possible movement restrictions.

Floppy skin overhangs, severe movement restrictions. Skin tightening often necessary.

How widespread is lipedema?

Lipedema is a very common condition. In the USA alone, 17 million women are affected, and worldwide the figure is estimated at 370 million. In Germany, up to 10% of all women suffer from lipedema. Despite clear symptoms, studies show that half of all patients wait an average of 10 years before receiving a diagnosis.

What causes lipedema?

The cause lies in genetics, with episodic changes coinciding with hormonal fluctuations. This is why lipedema affects almost exclusively women. Men can be genotype carriers but are protected from phenotypic manifestation due to the absence of female hormonal fluctuations.

What triggers lipedema?

Lipedema is usually triggered by changes in the female hormonal balance – in connection with puberty, pregnancy or menopause, episodic accumulations of fatty tissue occur.

Where does the pain in lipedema come from?

Since lipedema involves an abnormal accumulation of fat cells that multiply and enlarge over time but cannot reduce their volume, the typical symptoms of pressure, pain and heaviness occur. The increased breakdown of excess fat cells also leads to painful inflammatory reactions.
